Chronic Chest Pain After OHS/CABG

  • 1.  Chronic Chest Pain After OHS/CABG

    Posted 09-11-2024 14:50

Hi everyone, I'm a new member and hoping for some helpful input. I had OHS 5 1/2 years ago. I've had constant pain in the sternotomy region and in my pecs ever since. Sometimes the pain is minimal, but sometimes I get sudden attacks of severe pain that can last hours to days. As you can imagine, it's a scary feeling to have chest pains when you have a cardiac history. I've tried painkillers, acupuncture, heat, OMT, and physical therapy. Even with all of this, I still have constain pain and sudden attacks of severe pain.

None of my doctors is able to explain or resolve this. Has anyone experienced these symptoms? Is there a cure for this pain out there?

    Thanks for reading. Input appreciated.

    Raj,
Since coronary artery bypass graft (CABG) is the most common type of open heart surgery, your long-duration post surgery pain is quite unusual. Since you've tried so many other solutions, you may want to consider these two options:
    A. Keep a diary of the incidences of chest pain including date, time of day and any unusual upper body exercise or physical stress.
    B. Schedule a follow up consult with your surgeon to review the diary for any unique patterns or other physiological conditions.
    Best of luck,
    Vic

    Raj,

Several things could be attributed to the pain and discomfort you are experiencing. The sternum takes about one year to heal fully. Are you working out or doing any heavy lifting that might be causing the pain? Do you have a chronic cough or allergies? A competent primary physician should get to the bottom of this, but if you think it's related to your surgery I would seek a second opinion from a cardiologist.

    Hi Terry,

Thanks for your response. Its been over 5 years since my surgery, so I'm fully healed. I avoid heavy lifting, but lifting does make it worse. No cough or allergies. Would you believe that multiple physicians and cardiologists, all of whom I consider to be very competent, are unable to figure it out?

    Best,

    Raj

    Hi Vic,

Thanks for your response and well wishes. I actually started keeping a diary of the pain a few attacks ago to see if I can trace anything. I followed up with my surgeon about 2 years after the surgery. He couldn't offer anything after seeing a normal CT.

    Best,

    Raj

yes Raj, I have constant pain that sounds similar to yours. I am 9+ months out of mitral valve repair. also, it seems my left arm weighs too much as the shoulder will ache a ton after 15-30 min. of it hanging at my side.  i have to carry my arm with the other one for any relief. no one can explain it. i too have tried just about everything. there are two things that seem to help - massage and meditation.  these are temporary but am hopeful it will have cumulative effect. All good wishes Raj

    Hi Annie,

Sorry to hear that you're going through this too. I've recently started massage again after a long break. Hoping that it gives me relief again. Wishing you all the best and hoping that you will feel better soon.

    HI Raj,

I had a CABG x 4 in July 2022 and I've had sternotomy pain ever since. My cardiologist refers to it as "post sternotomy pain syndrome" and other than ruling out heart problems, he doesn't have any suggestions. My pain is minimal buy I am on 600mg of gabapentin daily, and I'm thinking of cutting back. Someone on this page mentioned monitoring your pain and have just begun doing just that.  See what your primary physician has to say once you have an idea and he/she should know where to go from there.  Best of luck.

    Hi Jeff,

I discovered post sternotomy pain syndrome myself earlier this year through my own research. None of my physicians or cardiologists ever mentioned it, but it seems like that's exactly what it is. You're right, cardiologists will just rule out heart issues, but can't do anything about the pain. Sucks for us. My pain can be minimal for weeks, but will suddenly flare up for no reason. Based on how it feels, I also have to rule out heart issues at times. Pain management has me on Lyrica. Multiple docs have no idea what to do besides pain meds. I'm just hoping for a miracle. Thanks for the well wishes and wishing you the best as well.

    Take Care,

    Raj

    Hi Raj,

One last thought and this may or may not be true - supposedly the wires used to keep the sternum in place during recovery are permanent. They could cause issues and are removed in some cases. I'm seeing my primary and cardiologist in about a month and plan on discussing it.

    Hi Jeff,

The wires are intended to be permanent. I found a couple of articles a few years ago which attributed the pain to the wires and talked about removing them. I talked to my cardiologist about it immediately. He was against it and did not support going back in. He felt another surgery could cause even more problems and probably wouldn't help the pain.

I would definitely be interested to hear what your docs say.

    Best,

    Raj
